Applications are peer reviewed for acceptance. A 1-2 page Professional Resume or Curriculum Vitae (CV) must be submitted for each presenter. Each presenter must disclose any conflicts of interests.
Presenter(s) must show evidence of appropriate educational preparation, experience, and qualifications related to the proposed presentation.
Lead Presenter Disclosure
All faculty/presenters/authors are required to disclose any and all potential conflict(s) of interest for themselves and/or their spouse/partner (owner or sole proprietor, speakers’ bureau, grant/research support, major stock shareholder, employee/paid consultant, etc.). All discosures that are determined by the Program Coordinator to be relevant relationships will be shared with the participants/learners in the meeting materials and prior to the start of an educational activity.
By typing my name below, I am providing it to represent my electronic signature approving all the information entered in this Call for Programs Form. I further attest that all submitted information is accurate. I have identified all potential conflicts of interest and for those conflicts of interest that could bias my presentation, I agree to abide by the resolution of conflict as determined by the Program Coordinator.
Co-presenters are individuals who are actively involved and whose expertise contributes to the presentation. Each co-presenter form must be filled out in its entireity, including resumes and CVs and disclosures. List only those people planning to attend the conference to present. All presenters must register for the conference.

Presentation Format, Topic, and Target Audience
Presentations must conform to the maximum time limit for the type of presentation requested as indicated in the Presentation Format Definitions. Handouts and time for discussion are strongly encouraged but must be within the presentation timeframe.
Programs promoting any commerical venture must be submitted as a Commerical Session and the product or service must be available in the Exhibit Area. Promoting a commerical product or service outside of a commerical session opens a presenter to sanctions as determined by the Board of Directors.
Presentation Content and Structure
Abstract, Learning Objectives and Content, Program Summary
Review criteria will include:
Learner Objectives should be stated in terms of participant outcomes and provide evidence of learning. Objectives should be realistic, measurable, matched with content and appropriate for an interdisciplinary audience.
Begin each objective with one of the following measurable verbs: Describe, Recite, Explain, Identify, Discuss, Compare, Contrast, Define, Differentiate, List, Review, Outline.
Make a separate objective for each action. Example: Define sleep deprivation and the consequences. These are two separate actions and should be split into two objectives as follows:
1. Define sleep deprivation.
2. List the consequences of sleep deprivation.
Content is the information needed to meet each learning objective.
Objective: Define sleep deprivation.
Content: degrees of sleep, quantity of sleep, quality of sleep, circadian factors
Complete this sentence for each learner objective (maximum of four), “At the end of the session, participants will be able to…”
